UIL Form
Physical Form English, Physical Form Spanish - This form will have to be filled out by your doctor and filed with the athletic department.
Honor Code Acknowledgment - Must be completed and submitted before you attend any band events.
RANK ONE
Rank One forms must be completed online. There is no option for band, simply select any sport. Blank fields will not be accepted. Please enter N/A on anything that does not apply. There are five sections to complete.
